Ontario Health atHome (formerly known as Home and Community Care Support Services) delivers publicly funded home and community care services across Ontario. If you or a loved one needs support at home, you may be eligible for free services through this program — and Bayside Home Care is here to help fill any gaps.
What is Funded Home & Community Care?
Funded Home and Community Care (FMHC) provides eligible Ontarians with a range of health and personal support services in their home or community setting — at no cost to the individual. These services are coordinated through Ontario Health atHome and delivered by approved service providers.
Eligibility is based on a clinical assessment of your care needs, not your income. Services are time-limited and aim to help you remain safely at home, recover after a hospital stay, or manage a chronic condition.

How It Works
Get a Referral or Apply
You can be referred by a doctor, nurse practitioner, or hospital team. You may also contact Ontario Health atHome directly to request an assessment.
Needs Assessment
A care coordinator will review your needs, which may include a phone or in-home assessment, to determine your eligibility for publicly funded home care services.
Care Plan Development
If eligible, a personalized care plan is created outlining the approved services, frequency, and duration of care.
Services Begin
Approved services are arranged through contracted care providers. Bayside Home Care can provide additional private support to complement publicly funded services where needed.
